　　(一)注冊(cè)會(huì)計(jì)師注冊(cè)申請(qǐng)表;

　　(二)注冊(cè)會(huì)計(jì)師全國(guó)統(tǒng)一考試全科合格證書復(fù)印件;

　　(三)2名注冊(cè)會(huì)計(jì)師出具的注冊(cè)申請(qǐng)人從事審計(jì)業(yè)務(wù)2年以上證明表;

　　(四)與所在會(huì)計(jì)師事務(wù)所簽定的聘用合同復(fù)印件;

　　(五)有效身份證件或者身份證明復(fù)印件(外國(guó)人應(yīng)當(dāng)提交護(hù)照和簽證復(fù)印件，香港、澳門特別行政區(qū)及臺(tái)灣地區(qū)居民應(yīng)當(dāng)提交在香港、澳門特別行政區(qū)及臺(tái)灣地區(qū)的身份證件復(fù)印件和中國(guó)出入境行政管理部門發(fā)放的通行證復(fù)印件替代此項(xiàng)材料);
